Community and independence

Clinical support
Community nursing related to disability needs, based on assessment and the participant’s plan.

Home and accommodation
Specialist disability accommodation for eligible participants where it is included in their plan.
Registration groups

The titles below match the client-supplied registration groups exactly. Whether a particular support is suitable still depends on the participant’s plan, needs and agreed service arrangements.
Daily living
Personal activities, shared living and household tasks arranged around day-to-day needs.
- Assist Personal Activities High
- Assistance with personal activities where more complex support is required, shaped by assessed needs and the participant’s plan.
- Assist-Personal Activities
- Assistance with personal tasks and daily routines at the level agreed with the participant.
- Daily Tasks/Shared Living
- Support with daily tasks in a shared living setting, based on individual routines and agreed assistance.
- Household Tasks
- Assistance with household activities to help maintain a safe and workable home environment.
Community and independence
Support with transitions, travel, skill development and participation in community life.
- Assist-Life Stage, Transition
- Support to plan and manage changes in life, with practical steps agreed around the participant’s goals.
- Assist-Travel/Transport
- Travel and transport support connected with daily activities, appointments and community participation.
- Innovative Community Participation
- Individualised support to develop skills, connections and participation in community life.
- Development-Life Skills
- Practical skill development for everyday life, chosen with the participant and linked to their goals.
- Participate Community
- Support to take part in social, recreational and community activities selected by the participant.
- Group/Centre Activities
- Supported group activities in community or centre-based settings, aligned with individual interests and goals.
Clinical support
Community nursing related to disability needs, based on assessment and the participant’s plan.
- Community Nursing Care
- Nursing support related to disability needs, guided by clinical assessment and the participant’s plan.
Home and accommodation
Specialist disability accommodation for eligible participants where it is included in their plan.
- Specialised Disability Accommodation
- Specialist housing for eligible participants where SDA funding is included in the NDIS plan.
